How do I remove this 140A fuse?

How do I remove this 140A fuse?
Folks, this fuse refuses to come out with a pair of pliers. If I try harder, I think the plastic is going to crack. Anyone know how to remove this fuse? Good place to buy a replacement?

You pull it out, as you do the rest of the fuses. I base that on the electric wiring FSM for my '01...
More important than how to remove / replace the fuse is WHY it blew. Any ideas?

Some of the larger fuses are bolted in from the side.
You need to separate the inner fuse box portion from the outer sleeve to access the bolts.
For some reason I remember one side being an 8mm and the other being a 10mm on one car I did.
I think they bolt them down because of the high current flow.
This is off memory but I've seen it like this on multiple vehicles.

Maybe going to a parts store and looking at a replacement might help? The electric wiring diagram book mentions nothing special about that fuse, which the book actually terms as a "fusible link".
