Default Help with leather refinishing

My leather is pretty good for a 10 year old car, but I have some cracking on the driver seat where your left shoulder and upper arm would rest in the seat back (this spot now has a slit that started about an 1/4 inch but is slowwwwwwly growing) and a little hole on the passenger seat where the seat back starts at the bottom (that one is worn from the hard plastic underneath rubbing it). I've got to nip these in the bud before the seats look like complete crap. Anyway, I was wondering if anyone could recommend a good leather repair kit that actually works and can be done by a non-professional (I think I can handle it?). Also, any tips on how to accomplish the best results? Any help would be greatly appreciated...thanks!
