I think speculating about based on personal experience and reputation it is kind of silly when one can easily access real data from large statistically valid studies. Consumers Union just tabulated results for 965,000 subscribers covering 1.3 million vehicles (see the link CAIFAN_64 posted above). J.D. Power and Associates does similar research. The nice thing about these studies are that they are broken down by car model and year, since not all vehicles by the same manufacturer score equally well.

What we don't know is how much difference there is in the top 5. Is it statistically significant for Lexus to fall from 1 to 5? Or are the top 5 so close that being no. 5 is no worse, for all intents and purposes, than being no. 1 or 2? Americans are too focused on ratings in everything we do.

If we're basing this on personal experience, for me Honda would be first, and Lexus wouldn't even appear on the list. But that rating doesn't do anybody any good. I think we should leave the speculating to companies that research/survey reliability for a living.

Depends on the source involved, what reliability level they are measuring, how they do the surveys, and how they record the data. According to Consumer Reports and J.D. Power, the top two organizations in the buisness, lately, Toyota, Honda, Subaru, Lexus, Acura, and Hyundai have been the most reliable brands in general....not necesarily in that order.
From what I have seen lately, Hyundai, in general, seems to be offering more quality for the money spent than any other brand. What they deliver, for the asking price, today is nothing short of astounding.
Of course, Toyota and Honda products have a long history of being at or near the top of the reliability charts, and despite some problems with late-model Camrys and Accords, and some quality drops on some new Lexus models, that in general is not likely to change much either. Subarus, except for some head-gasket issues with some 2.5i engines, have a long history of superior reliability records as well, and they have long done the best AWD systems in the buisness under 35K.
