It would be nice to see more details.
I'm guessing they want the dealer service department to look at the mats and make the determination as to which mats should be destroyed and replaced. Just a guess, but if that's the case not all mats will be replaced.

I doubt this but it would be nice. My g/f's POS OEM all weather mats got a hole the first year she had them in. We first figured it was from her driving with heels all the time but that's clearly not the case as many members have had this issue.
Another thing is I don't really understand that issue with the floor mats slipping under the accelerator pedal. Is it from not using the hook where the hole is? I currently have the all weather mats hooked in and on top of the carpet ones, which I guess you're not supposed to do (?), and I was pulling at it the other day and I don't see how or why this would happen...

Go to any car wash and watch how often they put the mats back in the car with the hooks engaged. I'd bet a whole lot of these incidents have happened after someone got the car washed with an interior clean and the mats were just tossed back in the car, not properly restrained with the hook(s) provided by Scion/Toyota/Lexus.

I have the all weather mats on my 06 awd and have not had a problem...need to recheck...most lexus mats get hooked in right? may these didnt get hooked in or came unhooked...

I'd bet the majority of the incidents we've heard about are after the car has been to a car wash and the mats were not properly installed after being removed for cleaning and interior vacuuming.

Toyota is recalling 3.8 million Toyotas and Lexuses because floor mats could make the accelerator stick, the National Highway Traffic Safety Administration has announced.

that is one heavy mat. i have the all weather floor mats on my IS and they fit perfectly underneath the pedals
