That one in the Bolivian Andes takes the cake.

We could (arguably) add the upper parts of the Pikes' Peak Road in Colorado to this list. It is unpaved, has drop-offs of many thousands of feet, and has no guard rails.

Those are some seriously crazy looking roads, my word. #5 the LA one I drive through that one all the time and if you don't know exactly where you're going will get twisted for sure.

What, no Mt. Akagi and the gutter technique? I'm disappointed.

All I would need to be in heaven would be a Ferrari 458 and one of those roads all to myself. (also endless tank of gas)

That's a good suggestion. Should have rounded out the list to top 20 lol.

After driving a Lexus for a few years you kind of lose that appreciation for driving feel and develop more of a appreciation for luxury. I dont think my GS would be able to do much on those roads but cruise calmly through them

Some great and treacherous roads, makes Pike Peak look like easy street.

I agree that some of those other roads may be scarier than the Pikes Peak road (the Andean Bolivian Highway of Death is perhaps the worst one of all), but the Pikes' Peak road is no picnic, which is one of the reasons why the annual Hill Climb is considered one of the most difficult drives in racing. That's why I said, in my suggestion, that the Pikes' Peak road could arguably be included in the list of Top 20.
The EPA has attempted, for several reasons, to close down the road a couple of times, but relented from local opposition (partly, of course because the annual race, and tourism, brings in money).

The Mt. Washington road in NH could also be on the list, but somewhat further down than either the Pikes' Peak road or the other roads in this thread.....I'd put it maybe in the Top 30. it is steep and somewhat treacherous, without guard rails, but lower in altitude than most of the other roads mentioned here.

I'll agree with Pikes Peak, took a motorcycle up a few years ago. 70 degrees at the base and below freezing on the peak. How in the world do they race on that road!! On a bike was frightening enough.

Yes, I drove it in 2006. The main problem was the lack of oxygen at the top, I've skied at 10K dozens of times, but 14K was really something.
