A global recall of nearly 2.9 million Toyota SUVs was prompted by a Canadian investigation into a crash that killed two people when their seatbelts failed in a collision.

In documents posted Thursday, U.S. safety regulators say the recent recall was prompted by information from Canadian investigators about a crash in Eastern Canada in 2011 in which a Toyota RAV4 and a Chevrolet Silverado pickup truck crashed into each other.

Front-seat passengers in both vehicles survived, but two rear-seat passengers in the RAV4 were killed.

That prompted Canada's auto safety regulator, Transport Canada, to do crash tests on the vehicles in question to try to see what could have happened.

Transport Canada posted video of their crash tests and how the belts failed


Transport Canada found that the rear seatbelts on affected models can come in contact with a metal frame during a crash, which can cause the strap to snap. Then the belts don't restrain passengers.

Transport Canada took this information to U.S. safety regulators, who conducted their own tests and found similar results.

"This is more than just a case of Transport Canada finding a safety problem and doing a thorough investigation," Transport Minister Marc Garneau said.

"As a result of the department's investigation into this safety issue, lives will be saved in Canada and around the world."

The recall covers RAV4 SUVs from the 2005 through 2014 model years, the RAV4 electric vehicle from 2012 through 2014, sold in North America, and the Vanguard, sold in Japan from 2005-2016, the Japanese automaker said.

Toyota dealerships can do a simple fix, adding plastic covers to the seatbelt's metal frame, at no cost to customers.

Ford Motor Company (Ford) is recalling certain model year 2015-2016 Transit vehicles manufactured February 13, 2014, to January 13, 2016 and equipped with rear passenger seats. The rear seat belt buckles may be damaged if a heavy object is placed on, or impacts, the top of the buckle. The damage may prevent the buckle from latching or from remaining latched.

CONSEQUENCE:
An unlatched seat belt increases the risk of injury to the occupant in the event of a crash.

REMEDY:
Ford will notify owners, and dealers will modify or replace the seat belt buckles, depending on their location in the vehicle, free of charge. Remedy parts are currently unavailable. Maserati North America, Inc. (Maserati) is recalling certain model year 2014-2016 Quattroporte and Ghibli vehicles manufactured February 1, 2013, to November 30, 2015. In the affected vehicles, the driver-side floor mat anchor may break allowing the floor mat to move and get trapped between the accelerator pedal and the vehicle carpet.

CONSEQUENCE:
If the floor mat moves and becomes trapped between the accelerator and the vehicle carpet, it may result in very high vehicle speeds, which could cause a crash, serious injury or death.

REMEDY:
Maserati will notify owners, and dealers will replace the driver-side floor mat and possibly the accelerator pedal cover, free of charge. The recall is expected to begin April 29, 2016. CONSEQUENCE:

TOYOTA RECALLS CERTAIN 2016 MODEL YEAR RAV4 AND LEXUS RX AND ES VEHICLES

TORRANCE, Calif., April 6, 2016 – Toyota Motor Sales, USA, Inc. today announced that it is recalling approximately 4,170 Model Year 2016 Toyota RAV4; approximately 90 Model Year 2016 RX350; and approximately 12,400 Model Year 2016 ES350.

The involved vehicles are equipped with a Brake Actuator assembly that enables enhanced brake control functions including the Anti-lock Braking System (ABS), Traction Control (TRAC), and Vehicle Stability Control (VSC), among others. There is a possibility that a component inside the actuator could have been damaged during manufacturing, preventing the actuator from operating properly. This could cause a loss of vehicle stability control, increasing the risk of crash.

All known owners of the affected Toyota vehicles will be notified by first class mail. Toyota and Lexus dealers will check the serial number of the Brake Actuator and replace actuators assembled during a specific production period.

Today, Tesla issued a voluntary recall for Model X built before March 26th of this year.
Here is the email we sent today to all affected Model X customers:

We are emailing to inform you of a proactive action Tesla is taking to ensure your safety as a Model X owner. Tesla's internally conducted crash testing demonstrates that Model X will be the first SUV to receive the highest safety rating in every category, and we are committed to ensuring that it remains the safest SUV in the world.

Recently, during an internal seat strength test that was conducted prior to the start of Model X deliveries in Europe, the recliner in a third row Model X seat unexpectedly slipped. The recliner, which is provided to us by an outside supplier, is the locking hinge that allows the third row seat back to fold forward, and if a recliner were to slip during a crash, the seat back could move forward. Similar testing was conducted before the start of deliveries in North America, with 15 confirmation tests having been conducted without a single recliner failure. Despite these prior successful tests and no reports of a third row seat slipping in any customer vehicles, we have decided to conduct a voluntary recall as a precautionary measure and will be replacing all affected third row seat backs.

Our records show that you own a Model X affected by this voluntary recall. We will shortly send you an official recall notice by mail, but we wanted to alert you to this action as soon as possible.

A fix to this issue is already in place. We have worked with our supplier to develop a new recliner design with improved quality that resolved the issue. We are now constructing new third row seat backs to match all affected vehicles. Your service center will contact you to schedule the installation of your replacement seat backs as soon as they are available. Based on current production rates, we expect all replacements to be completed on a rolling basis over the next five weeks. We will accelerate this timing if possible.

Until the recall is performed, you may make full use of your Model X, although we ask that you temporarily not have anyone sit in the third row seats while the car is in use. We recognize that not having the use of your third row seats for the next few weeks will be an inconvenience, but your safety is our primary concern. We assure you that we are building your new seat backs as fast as we can.

TOYOTA RECALLS CERTAIN 2016 MODEL YEAR AVALON AND CAMRY VEHICLES

TORRANCE, Calif., April 13, 2016 – Toyota Motor Sales, USA, Inc. today announced that it is recalling approximately 16,880 Model Year 2016 Avalon and 41,630 model Year 2016 Camry sedans.

In the involved vehicles, the front passenger seat is equipped with an Occupant Classification System (OCS) which activates/deactivates the front passenger air bag system, depending on the weight of the occupant. There is a possibility that some vehicles may not have received proper OCS calibration during the vehicle manufacturing process. With the improper calibration, under some conditions, the front passenger airbag and the front passenger knee airbag may not deploy as designed in a crash, increasing the risk of an injury to a front seat passenger.

All known owners of the affected Toyota vehicles will be notified by first class mail. Toyota dealers will re-calibrate the Occupant Classification System (OCS).

CONSEQUENCE:

GM Recalling Certain 2014-15 Pickups to Repair Seat Belts

DETROIT – GM is voluntarily recalling 895,232 model year 2014-15 Chevrolet Silverado and GMC Sierra 1500 pickups in the United States because the flexible steel cable that connects the seat belt to the vehicle can fatigue and separate over time as a result of the driver repeatedly bending the cable when entering the seat.

This issue was discovered through warranty data, and there have been no reports of crashes, injuries or fatalities related to this issue.

For trucks in the field, dealers will enlarge the side shield opening, install a pusher bracket on the tensioner, and if necessary, replace the tensioner assembly.

The recall in the United States includes a stop-sale of approximately 3,000 new 2014 and 2015 model year pickups still on dealer lots. Dealers will repair the vehicles prior to delivering them to customers.

Customers will be notified by General Motors. They also can look up their vehicle identification number (VIN) at either https://vinrcl.safercar.gov/vin/ website or at https://my.gm.com/web/portal/ownercenter to see if their vehicle is part of the recall.

GM also will voluntarily recall and repair these models in other countries where they are sold, including Canada, Mexico, and several countries in Latin America and the Middle East. The total number of trucks to be recalled globally is 1,037,982. The cost of the recall is not expected to be significant and is covered within normal and customary warranty reserves.

Fiat Chrysler is recalling 1.1 million vehicles worldwide to address the problematic shifter used on cars with eight-speed automatic transmissions. The issue is that the console-mounted shifter acts like a rocker switch and always returns to the middle position after moved. This has been deemed confusing to drivers – confusing enough to cause some to exit their vehicles without first selecting Park and leading to the car rolling away.

FCA says 41 injuries are related to the shifter problem, and no evidence of equipment failure has been found. The company will enhance warning chimes and alter the shift strategy, meaning alert messages will be displayed in case the driver door is opened while the engine is running. With the door open, the transmission will prevent the car from moving even if Park is not selected.

The affected vehicles are certain model-year 2012–2014 Dodge Charger and Chrysler 300 sedans, as well as model-year 2014–2015 Jeep Grand Cherokee SUVs, an estimated 811,586 US vehicles in total. The recall also affects 52,144 vehicles in Canada, 16,805 in Mexico, and 248,667 vehicles elsewhere. The shifter is used with ZF-designed and ZF-built eight-speed automatic; Audi uses a similar shifter setup in some of its vehicles, including the current-generation, which predated Chrysler's use of it.

Chrysler uses a different, a rotating-dial-type shifter on eight-speed-equipped Rams. The company moved away from the problem shifter design in 2015 for the Charger and 300, and the Grand Cherokee's shift lever was modified for 2016.

Owners of affected vehicles will be notified of the recall when service is available. Fiat Chrysler urges customers to follow the instructions in the vehicle's owner's manual in the meantime.
