What's fascinating about them and you know actuaries are behind it....
I stupidly bought one on a BMW 335i. Unreliable, CR most unreliable and avoid lists (then recommended for 2010 same car no changes) blah blah blah (turned out not to be the case). It was very straightforward, choices were 6/70, 6/100 (typical), 7/70, 7/100, from the date of in-service. Can be purchased up to the last day of the 4/50 warranty. Of course nothing broke except VANOS.
WHY WOULD ANYONE CHOOSE TO BUY IT DAY 1 AND FINANCE, when you can buy it the last day of the 4/50?
Now, many extended factory warranties are done say 3/54 FROM THE DAY IT'S PURCHASED. So let's say you buy it with 2 yrs. left on the factory warranty, it's written as 3/54 from that date and mileage, so essentially 2 of the 3 was already covered by the original warranty. Why are we as American consumers so easily snookered? I get that insurance has always done that, like when you insure say an engagement ring for 20k based on the appraisal, yet it's really worth 6k wholesale and that's all that insurance will pay out, they base the premium on 20 lol

What year is your 4Runner?
Seals and gaskets can deteriorate due to time more than just miles. If it's over 10 years or so, a leak like this wouldn't surprise me.
Edit: I see in your signature it's a 2004.

I was just thinking about this today while in Costco, they had an extended warranty on tvs, 3 yrs. for $60. Not bad, of course, but we have a 2013 Samsung now 4 yrs. old and nothing ever went wrong....so would not be useful....

Seals will leak, it's inevitable. Some will leak earlier than others, some will while you own the car or for the next owner. The ones that connect dissimilar materials will be more prone to leaking as will the ones that are exposed to extreme temp fluctuations or weather.

265,xxx mi from a 95 camry v6 1MZ-FE... No oil leaks from engine or auto trans.
Rack leaked at 155,xxx and the radiator cracked at 200,xxx. Two timing belts, oem water pump made it 21 years. Same with alternator and starter. Both hung in to about 250,xxxx mi.
ATF can leak from high up and mask itself. Find the source before labor is paid out.

Dude the car is 13 years old with over 100k miles on it. Its going to leak or have some sort of other mechanical issue due to the age, miles, just normal wear/tear. What makes the difference IMO on older cars is how often said problems happen, how easy they are to fix, how much $$$ OEM parts costs, are there cheaper aftermarket parts, and do said cheaper aftermarket parts actually last.
Main thing I look for when buying older vehicles, is how well were they designed from the factory for servicability and durability, and did the OEM actually spec good quality components??? Looking at some model BMW's on the forums, they are way too complex, this makes them hard to work on, and the OEM parts are brittle plastic junk that breaks after a few years because BMW wanted to save a few penny's per car.

I wouldn't be surprised there is a leak. One cannot determine longevity and durability based on mileage alone. Consider the age of the vehicle and location it has been driven (harsh elements, snow, salt, etc), Based on what the photo shows, there appears to be a fair amount of wear and tear underneath.
Hope it's not too major and resolved successfully.

The little 1MZ-FE was a good engine. To bad they detuned it as they did but they can't have it competing the family Badge! Things are still everywhere you look!
Valvoline dino oil and STP as it has flat tappet cam.
