i was able to install it on my sister car is a 2017 lexus NX but i cant hear notifications from on apple carplay like waze map or anything or Bluetooth people can hear me but i cant hear them mine did not come with an external speaker, was i suppose get one? because it was not in the box, thanks

You need to switch to AUX in Lexus menu. Lexion only works with AUX and same with phone calls you can only hear the other side from AUX input.
Found another nasty problem with Carplay not sure if it's iOS 15 related or my unit since I've never had this issue. When you're listening to music Waze the announcement audio will screw up your music. Music plays fine at first but after any audio announcement from Waze, music would come back on but it sounds like garbage. The only way around is to bring up Siri from phone. I cannot activate Siri after this happens from steering wheel control either.
Im on iOS 15 too, are you having an issue where you would be fullscreened on music and it would start shifting over to your contacts or maps? Mine keeps doing it, not sure if its because my wiring is loose or just the OS.

I am on the latest iOS 15 update and am having no issues. Thankfully.

I don’t have this problem doesn’t sound like typical Lexion issue I would check the connection.
