2012 with Cylinder 1 & 3 misfire but randomly
2012 CT200h with 220k miles
So had the check engine light and codes pop for 0300 and 0301 and 0303.
1. Replaced spark plugs, and injector coils, and same as before but not as severe and definitely far more random with the misfires
1a. also pulled and cleaned egr pipe, it was partially clogged and got into both receiving ends as much as possible, so it is now like new. didnt solve this particular problem
2. Misfires only occur when gas engine starts, either in the garage, or at a store, or a random stop sign or stop light.
3. Misfire refuses to occur regularly or with frequency. it is random, only consistent thing is it only does it from battery to dual engine running. it never does it when the vehicle is already under power or rolling, and pushing the pedal will mitigate it super fast.
4. No temperature or driving style will force this misfire (drive 75 miles and might not do it and then start up and drive 2 and still nothing but then start up and it randomly occurs)
5. It is not always both cylinders, #3 & #1 mostly and an occasional only #3 and a very random solo #1, but never 2 and never 4. Moving coils and plugs does nothing.
6. tried running the gas engine and unplugging the mass airflow sensor and car sputters but stays running, so not sure if that is any indication of a bad mass air flow sensor on this engine setup or not.
7. ZERO coolant loss over the time this has been happening. Both oil and coolant are clean as they should be, and the car does like to consume the 0-20 oil so now running 5-20 due to mileage.
8. I have to reiterate, that this refuses to misfire consistently and not every time it does misfire does it trip the engine light...
9 and trying to run fuel system cleaner in every tank and sticking to a single gas station with only high octane being put in.

Anything you receive here would be speculation, so I'll speculate away, but what you really need is a leak down test. This will allow you to see if you're leaking past intake/exhaust valves (indicating valve seat wear, valve where, valve steam seepage, carbon buildup) or through the coolant or crank case (both the latter indicating a headgasket leak)
At 220k with these cars known for HG failures at high mileage, you may be looking at a slowly failing HG. Engines contract and expand based on temp, so this may be why it doesn't happen consistently. Sometimes the HG seals, sometimes it doesn't.
Just my guess.
