I'm pretty sure the one in my '04 is original (157,000 miles) because the service history magically disappears at 89,000 miles on the Lexus site.
I'd guess a previous owner got spooked at the cost of replacement and dumped the car. I did crack open the cover, belt "looks good" but that is a poor indicator of condition and I'll replace when (if?) I get the motivation.

I replaced mine on my 2002 es300 last year. It had been 10 years, 60k miles since the last change. I also did all the seals, and water pump at the same time. The old belt literally looked brand new still, no signs of wear. I'm still glad I changed it though for peace of mind. I live in AZ, a warm dry climate.

Keep this in mind:

- Only Lexus dealership or owner-added services are included there. Even work done at a Toyota dealership is not entered.

Look around for a sticker; any decent mechanic will note a t-belt change with a service sticker somewhere under the hood. If you can’t find one, and it looks like it’s never been done - and you can determine that by examining bolt heads necessary for service - you should really think about getting it done.

Yes, the bolt heads for the cover look pristine and untouched which leads me to believe that the belt wasn't done. Oddly though, they replaced all other belts and hoses so maybe they were in there after all but of course that's mere speculation. I'll post if the belt breaks!

Unfortunately, those stickers can be bought:
