Need to ask a couple questions to get a better picture of the problem. Do you get any check engine lights when the transmission starts acting up? When was the last time you had your transmission fluid changed?

Default Shift solenoids may be the problem

This pretty much sounds like one or more of the shift solenoids inside your transmission are malfunctioning, causing mis-shifts and hesitation behavior of your tranny. I experienced the same symptoms in my '01 ES several months ago. If your ES is still driveable, I would go to Autozone and have the engine codes scanned; it is a free service. This will tell you what the problem is with your transmission, and from that point on, you will have to decide if you want to go to Lexus dealership, or an independent mechanic to diagnose the problem. Keep us updated.

The code scanners have a digital readout of the error code, usually in the form of "P####" (the # stands for a numeric digit). Also, if your ES is '96 or newer, it should be OBD-ii compliant for engine error codes. Check out http://www.obdii.com for more info.
