Same as the 90k service, change all the fluids especially the transmission and coolant. I'd also exchange brake fluid. I'd get a tune-up, with new plugs and wires. Change the PCV(Positive Crankshaft Ventilator) valve. Change the engine and cabin airfilters if those have more than 15,000 miles on them. Check all the rubber hoses for cracks. I'd also dump in a bottle of Redline SI-1 fuel cleaner in the gas tank to clean injectors and depending on how clean your engine is, I would also do a couple of cycles of auto-rx to clean your engine of any sludge of varnish. I think I got everything.

You should also have the timing belts and all the other belts replaced too if that hasn't been done yet.

Timing belt & water pump are due. Valve covers, plugs, flush fluids. Replace the tranny pan gasket if its leaking. Clean the EGR Valve and IAC valve.

If you can get it in the air, check the flex pipe, cv joints, motor mounts, and check for leaking struts. Take a look at your sway bar bushings and end links.

180K isnt a lot for a well maintained Lexus.
