Power Steering Hoses

Well I've had a leak in my lower pressure hose for quite some time now but today i think it finally met it's end. I pulled in my the parking garage and got into a spot and I started getting a lot of smoke for the engine bay. Powered off the card and popped the hood and it was coming from power steering fluid getting all over the engine. From what I can tell, fluid was only coming out of the lower pressure hose because of where it was located (passenger side, mostly near the rear of the engine bay), but I can't tell for sure. Heres the kicker though, I partially refilled my power steering reservoir, let and run an give the wheel a couple good spins and I didn't notice any excessive leakage, no spraying, and just some slight drippage, which until I replace the hose is normal.

Heres my questions:
1. What do you think the cause of all of the fluid inside my engine bay was from?
2. Is swapping out the lower pressure hose difficult?
3. Should I also replace the high pressure hose since I'm swapping the lower pressure?

I'm having the same issue right now. I can see a leak in two places: one is from the hose that goes from the p/s filler to the pump. The other leak is in the middle of the rack and pinion set, it seems to be dripping from there and it goes on the exhaust and starts to smoke. I have a 1999 with 225k miles. Not sure what i'm going to do yet.
