92 ES300 Failed AirCare in BC - Sensor Problem?

Failed aircare 2x in BC recently after installed new cat, new y pipes, 2 new o2 sensors, new plugs, new oil, replaced intake gasket, replaced MAF hose, new rad.
high CO- 34.75
high HO- 1.21
nox- passed
CAR HAD BLOWN RADIATOR LAST YEAR & OVERHEATED QUITE A BIT. all repairs to date done afterwards.
Could it be the EFI coolant sensor was damaged?
What about the H20 coolant sensor on side of engine?
Maf hose had big crack prior to changing so maybe MAF sensor is dirty?
No codes are showing up on CL.
Any ideas?

keep in mind sensors dont always have to show up bad on the cl all the time. the system can miss a bad sensor & not report it on the dash.
