Default Need the pros on this - Car throwing codes, need to try your suggestations

Hi everyone first time poster but long timer reader. I've tried to figure this baby out by reading/searching but no one seems to post back when they fix their problems.
Bought a lexus ES300 1999 had to replace the catalytic converter (not original) as it wouldn’t pass emissions. I think this is where everything started. The problem starts when my car is driven for about 10minutes, I would notice the car idling rough and burning rich, so the front catalytic converter would glow. Also when trying to stop the brakes would fail, also when the car is idle the RPM would gradually go up, I think it’s trying to compensate for the rough idle so it would let the car to shut down.

Here are the codes, it's pretty similar to some of the guys in this forum:

1. P1351 (variable Valve timing / CM Sensor Range/ Performance)
3. P0306 (Cylinder 6 misfire)
4. P0304 (Cylinder 4 misfire)
5. P0302 (Cylinder 2 misfire)
6. P0300 (Random/Multiple Cylinder Misfire)

These are some of the things that I’ve done:
1. Replaced the "front" spark plugs, Closer to the radiator, the others could not get to yet.
2. checked the OCV and it works by hooking up a 12V
3. Checked the OCV filter and it was very clean.
4. Cleaned the MAF sensor with alcohol (it wasn’t that dirty)
5. Checked for any tubes out of place, and everything looks in place.
Some things that I could do but before doing them I like to get some better ideas.
- Change the Coils and wires
- Replace the spark plugs in the back
- Change Sensors (but which ones)

Bank1 – Sensor1: O2S Output 0.790V
Bank 1 – Sensor1: STFT 0.0%
Bank 1 – Sensor2: O2S Output 0.025V
Bank 1 – Sensor2: STFT 99.0% <------DOES THIS LOOK RIGHT?
Bank 2 – Sensor 1: O2S Output 0.625V
Bank 2 – Sensor 1: STFT 0%

Fix the P1351 and it will solve your problem. Cat is glowing cuz theirs too much raw fuel in there so it's cooking it up. Change that OCV, it's a common problem. I know it's hard to understand, but applying voltage and seeing the solenoid working doesn't necessary means good. You must change it. Bank 2 is the one pointing towards the radiator.

Oright it seems that there are two OCV's the only one that I checked was the Left. But I'm not sure if the code is for the other OCV. There is a Left and Right, to make sure I understand - the LEFT is usually the raidator side and the RIGHT is closest to the firewall. Bank 2 is the one that i checked, does the code 1351 mean bank2 OCV problem?

Also Cylinder 2, 4, 6, are they the ones towards the radiator(LEFT) or the firewall RIGHT)
I've also attached a diagram picture to make sure

The front of the engine points towards the right-hand front wheel (passenger side) and the rear towards the transmission case (driver side). So the radiator is located on the left of the engine and the firewall on the right. Bank 2 (cylinder 2, 4, 6) is the one close to radiator (Left) and Bank 1( cylinder 1, 3, 5) close to the firewall (Right).

For peace of mind change both OCVs.

Thanks MikeLex - i'm ordering the parts for both Left and Right and see what happens.

So before ordering the Bank 2 OCV, I unplugged it to see what other symptoms would show up. i erased all the codes and drove around, the only code that was reported was that the bank 2 OCV is not functioning. No misfires or any other codes. So it showed me that this could be the root of the problem.

I got the part from Lexus for about $135 CDN (btw Toyota and Lexus parts are the same price here in Canada) and installed it. Drove around for about 15 minutes and everything seems great.
