Have you checked for spark, air, and fuel? Maybe even compression, could be a bad head or your camshaft is off on timing. Has the timing belt been done recently?

Yes - the timing belt was replaced a few thousand miles ago. What should I check?

I'm afraid that this is one of those problems that put a challenge to any automotive expert. At first, I thought that you could have been facing a defective OCV - typical issue for the first generation of VVT-i 1MZ-FE engines. Once you have ruled this out, I'm really curious to know what you will find out. Please, keep us posted.

Would you say that the issues only started after the timing belt change? You might be off a tooth on the belt.
