Lexus Es300 Runs lean right after refueling

Hey guys,
I have a 98 Lexus Es300 with 163,000
It runs lean, for a little bit right after refueling and will occasionally throw a bank 1 running lean code, . Usually it will shudder a little bit and not run as smoothly. Today for the first time right after I filled up the tank and pulled into the road it shut off. I restarted and it started right up and drove fine even on the freeway.
I am not hundred percent sure but I think maybe this happens more often after I have used fuel injector cleaner. Could it be that there is some gunk in the tank and after the fuel treatment it loosens up and temporairly clogs the fuel pump strainer?. Has anyone experienced this before?
Also I have already replaced the air fuel ratio sensor and have tested the MAF and both Air Fuel Ratio sensors as well with a elm 327 and proscan diagnostic software.

Like you said, you might have some gunk that gets lifted from the bottom of the tank, every time you refuel. Remove your fuel pump assembly and take a look, your fuel pump strainer might be clogged.
It's very easy to do. Remove the rear seat cushion, peel off the metal cover and unclip the electrical connector, then unclip the fuel line (get some rags to soak up any fuel). Then all that's left is some 8mm bolts and you're ready to lift up the fuel pump out of the tank.
