Well I have the 3 coil es300 but I know it will throw an igniter code if I unplug one of the coils connectors, this could be relevant in the fact that a bad coil/unhooked coil causes the car to throw a igniter code or it couldn't matter at all. I would move all coils from the front to the back and see if the issue(s) changes. Bad plugs, coils, or connectors could cause this...outside of a bad igniter.

Well that leaves you with bad connectors or a bad igniter. I'd try the connectors first. Here's a video on how to change them I'd bet they're cheap to pick up from Toyota.
I'd change those connectors, yea it's work but still. Maybe you could get at them without pulling the Plenum.

final result, and after a week in the shop. Cylinder 6 fuel injector was dead. Found this out after replacing the igniter and that wasn't the problem either. Mechanic seafoamed for the other 2 injectors.
So if you have this code, and it isn't the coil, it isnt the plug, and still aint fixed. Look at the injector.

Spark, Fuel, and Compression - that's what I try to remember whenever I get stuck.
