I cant believe this
so on my day off today and decided to change my spark plugs and boots.almost 1 year my check engine light was on so i go to a toyota/lexus parts dealer 10 mins away from my house,this was a every 3-6 months deal i would go and ask for the codes for the check light.i got the same answers this code is not in the system or just make sure you have a tight gas cap after you pump,these were the answers i would get everywhere i would go.so i just left it knowing somethings not right,anyways im changing my spark plugs the front 3 were EASY of course but the back 3 were HELL i didnt want to remove anything extra just for some spark plugs.the two outer ones were a little harder and took more time but man the middle one was a beast man.so i unscrew the boot and remove the boot i stick in the socket and i hear something snap on to the socket.my first thought was WTF,i pull out the socket and the spark plug was in the socket,i didnt even unscrew the plug a bit.in the third pic the top spark plug is the one that came out and i notice it didnt have the ring around the bottom. couldnt find it at all,after all that shock i put everything back together and take a drive on the highway and in the city and no check light and drives a whole lot better.Crazy things

Cliffs Notes:
1. He had a CEL for over a year that no one could diagnose (not even a dealer)
2. He changed the spark plugs and found the center plug on the back bank wasn't installed properly (it's a tough job as everyone knows)
3. he reinstalled new plugs properly and everything is now cleared (he's happy, and exasperated no one could figure that out after a year of trying to diagnose it)

Sorry, can not buy this one. You telling me you did not have a P0300/P0303 code? You were shooting raw gas into your cat and did not burn it up? No downstream O2 DTC? You did not smell any raw gas for the year you drove this way?

so the question is what happened to the missing crush washer.
if i remember correctly they are captive, meaning you cant just slip them off/on.
(someone correct me if im wrong.)
if they are in fact captive, that means your engine ingested it which could mean
possible damage to your valves, piston, ring, etc
spark plugs do not require a lot of torque.
and for those that dont have a torque wrench, NGK suggests tightening them hand tight,
and then one extra 1/3 turn or something similar.
also, you DO NOT want to use anti seize on your spark plugs, NGK issued a tech article
about that.
oh and lastly, dont install/remove spark plugs from a warm engine.
