2000 ES300 Not shifting into 4th gear?

My son's car does not have any CEL or other issues that I am aware of, it just will not shift out of 3rd gear. RPM's stay above 3000 or 3500 (and climb) at any speeds past 45-50. I am scared to even try highway speeds for fear of over-revving and causing damage. Any ideas on how to fix this?

The car will not shift into O/D until the engine reaches a certain temperature. Is the car heating up properly?

Yes. I drove it extensively one day (>30 minute one-way commute with in-town, stop-and-go traffic with red lights) and it never shifted into 4th gear. Speed limits reached 60 mph and the rpm gauge made me nervous so I kept it around 55.

If you had a OBD II tool that allowed for live data you could check to see if the reported A/T temp is reaching normal operating temperature. It is possible for sensor to malfunction or the sensor not to sense the actual temp for some other reason. The computer will not allow the vehicle to shift to O/D if the temp is too low.
That being said my 2001 repair manual suggests the following:
-- O/D main switch circuit (basically checking the O/D switch is working)
-- O/D cancel signal circuit (basically checking the cruise control)
-- ECM
