I dont know how situation with Lexus es350 transmissions, but usually the following are the choices:
1. Re-manufactured transmission swap from Lexus. This is the most expensive one. Re-manufactured transmission is equivalent to new as its rebuilt by Toyota authorized facility. You can only buy this at the dealer I would imagine.
2. used transmission swap.
3. re-building existing transmission by taking it apart, and fixing everything that is wrong with it.

I know I am going to sound rude and forgive me (also please do not reply: "it's just a Camry".
It really sucks owning a Lexus living in a town with 3 mechanics 2 of which are also a bait and tackle shop and a bail bondsman.
It may be worth it for you to tow it to a larger city with a reputable Asian shop or a L Dealership....

agree, the job better be done right. You might save some money but if it breaks again, it will be a waste.

FWIW: I knew a guy who bought a Transmission repair franchise. 100% of what he knew about transmissions was this: Transmission repair is not rocket science, but it is a super- secret black-box item to 99% of drivers, and thus, a very high-profit business.
Turns out that *typically* a high volume transmission repair shop keeps in stock, literally on the shelf, up to half-dozen of each of the most popular transmissions they rebuild, the out ES350s and same vintage V6 Camrys were popular, so swapping out your trans for a rebuilt is a pretty quick process. Going to nearby Big City for a "rebuild" may be a good choice. BTW, Fred told me the difference between the various lines (Sometimes Gold, Silver, Standard) is almost all about the length of the warranty, the quality of the materials & labor in the rebuilt units are virtually identical.
I'd buy from a high volume trans shop, given they have a decent (12 month minimum) warranty, as well as a decent Rep with local BBB.

Curious what the owner ultimately decided to do. Expensive repair or trade up to a different car? In this market it may actually make sense to fix it if the rest is in good shape.

I wouldn't trust a rebuilt. I would trust a used transmission from a known donor vehicle, ideally with low miles and good maintenance history. It's harder to find a good, used tranny, but worth the effort.
