Hesitation when accelerating

Late to this thread but I experienced something similar with my 2013 GS. Brought to dealer, all things checked out. I paid close attention and realized if I was two footing it, that caused it. I was able to recreate. So if I hit the gas before brake was fully released the car really hesitated, like limp mode style. I’d have to double tap gas to go. For me as above it was the brake not being released fully when two footing it. Try it and see if that’s the cause.
You shouldn't be two footing a car on the road anyways.
One thing people might be experiencing assuming they are not two-footing is Lexus vehicles have a pretty lazy electronic throttle system. Theres no throttle cable, which is typical today but if you're used to a car with more immediate throttle response it can cause a little perceived hesitation sometimes. I've been driving them so long it doesn't bother me anymore, but it did when we were first introduced to the brand.

Does this seem to happen primarily when the car is "cold" or having just recently been started from (for example) an overnight parking or an extended time parked so that the entire car is essentially "cold"?

Perhaps the hill lock is being engaged? If the brake pedal is pressed firmly the brakes lock and the car will not move if the foot is lifted off the brake pedal and the gas pedal is pressed firmly.
