Also can hear it if I shake the steering wheel left to right real fast when parked. Mainly does it only when wheels are straight. Any ideas? Drives and functions perfectly. This was on a gravel road to make the noise more pronounced. Seems to be close to the U joint inside the car and tied to the steering in some way. I’m stumped.

I also have this problem. Rattle-like noise when driving over pavers, and also over reflector in the road like HayHay reported. Not a loud noise. Seems like I can almost feel in from the steeting wheel. Sounds close, "inside" the dash, not in the engine compartment. No noise when driving on roads at all speeds. (We have generally smooth roads here in south Florida.) Just started happening a couple weeks ago after Lexus dealer replaced the plugs and gave the car courtesy check. I plan to have the dealer check it when I have a chance. Hope there will be no charge.

Update on my rattle under the dash problem: The lexus dealter wanted $199 to diagnose the problem. They woudl not speculate what the problem could be. After reading here that a dealer repair cost would be in the thousands, I decided not to do it. I can live with a little ratting.
Because I was getting some rattling at highway speeds, and it had been 3 years since my last alingment, I took in in to tire shop and had an alignment and rebalnced the tires. Much improved. No rattle on the highway. Much less rattle on other roads.
Next, I found a new shop nearby who are specialists on Lexus. Called them and they were familiar with the problem. $65 diagnosis charge. 10 minutes appointment, and they confirmed. The noise is from the bushings in the power steering motor under the dash. They explained that Lexus dealers only offer a $3k fix. But, Toyota dealers will fix it with a repair kit for much less. The shop has found that this repair kit works on the Lexus. They quoted $600 to fix it with this kit. 2 hours of labor, because the steering wheel and everthing needs to be removed. The said the fix would remove all traces of the rattle, and would be a long term fix. Since I don't drive this car much, and because the alignment fixed much of the rattle, I decided not to spend the $600. The car is 10 years old and has 120k. If it gets worse I will have it fixed.
The real bad news: I asked if Lexus fixed this Denso motor in the past 10 years, or does Toyota/Lexus use the same part. Answer: No, Toyota and Lexus use the same motor and have not fixed it. It would be a simple and cheap redesign to put in better bushings. Boo, Toyota and Lexus! Fix your damn part!
This is my first repair item on this ES. other than one failed parking senor. It's a shame to have such an expensive repair on such a simple part that should never have failed.

I used part 45254-28040 to do the repair on my 2013 ES350. I bought this part from a Toyota dealer. It's the same part whether you have a Toyota or Lexus. It's not a difficult job as long as you have some basic skills. The rattle noise is completely gone now. If this one lasts another 10 years I'm ok with that!

Did you take any pitcures, or have any instructions or advice when you did it? How many hours did it take? Or did you just go for it? Can you recommend how I would begin, or what tools I would need, or any youtubes that might help? I have no idea how to take apart the steering column

Any way to test if this is the problem or not for other people having a rattle in the same area?
I have a hard to diagnose rattle on the driver's side, almost sounds like it's coming from the dash. It's intermittent, seems temperature and humidity make it come out.
I'm thinking it might be a strut mount, but don't want to throw parts at it.
