I have the T Box Plus and not sure about the Max but what I did notice is that only the usb port closest to the front of the car will work. It does not like going through a hub and this will work with any car that has apple car play. Not sure about the ottocast but a bluetooth mouse works perfect with the T Box Plus. Did you try that usb port?
I don't even use android auto wireless with it as the whole screen is an android tablet. Takes about 30 seconds to load and does support split screen.

Forgot to mention that the usb c connection to the box is tight and has to be pushed in all the way like an extra 1/16 of an inch. Have read a lot of people don't push in all the way and also happened to me at first.

So this device plugs into the USB port and then works with the phone that is connected via bluetooth?

Yes it actually has 2 bluetooth connections so one can be used with mouse or keyboard. You can use wireless android auto with it but don't have to as it is a tablet now and your phone can be paired for calls etc. Your phone does not have to be connected for this to work

Well how can it project the phone if the phone is not connected? Or is the phone connected to the device via bluetooth?

It does not project the phone. It only projects the phone if you use the android auto wireless capability. This is a stand alone device that turns the screen into an android tablet. You don't have to use phone at all if don't want to but you will most likely pair it for calls etc. Also use phone as mobile hotspot for device

I tried every USB port and even tried with a different wire. Was so hopeful for it to work but nada . Maybe I just had a faulty one? Regardless, I had already returned it. The ottocast didn't have a bluetooth mouse.

The more reviews I read on Amazon, the less likely I am to purchasing. Looks like Amazon warns the product is often returned and there are widespread connectivity issues.

Is it worth $160? Absolutely not. Is it worth $100? Definitely a better option than buying a new iphone. I've also considered getting gromline, which is supposedly seamless, but you have to take apart the entire thing to install it and including cost of labor it's about 1,000-1100.

That's the good thing about an amazon purchase is being able to return easy. As for connected issues I have none but don't use wireless either. I have heard Lexus wireless drops connection also. I purchased mine directly from the carlinkit store in China to make sure I didn't get a counterfeit one. They only support devices purchased from there store. Got mine in Dec 22 and even though no problems there have been 3 software updates. There is tremendous upside if it does work especially for a vehicle that Lexus refuses to update software in US for android auto as they did overseas. Paid $150 for mine.
