It's an indication of how well you play golf.
Example:
I play to a 12 handicap, which means on a par 72 course, on average, I'll shoot an 84. Other factors come into play such as the slope and rating of a course. A low slope/rating par 72, I may shoot 80. A high slope/rating par 72, I may shoot 90. Both could still keep me at a 12 handicap though.
