Has anyone ever caused an accident due to their stereo or video system?

I just installed my new Clarion head unit. The very first day after I installed the unit and the video bypass. I drove around with a movie playing... a movie I have seen a hundred times. There was some stop and go traffic on the highway so of course I was glancing over to the screen. I nearly rear ended a guy because I took my eyes off the road. I vowed that will be the last time I ever try that...
Fast forward to yesterday. I show my girlfriend where I mounted the video bypass switch and now she wont friggin pull out of the garage without popping a DVD in! Even when she is the only one in the car...
I am about to relocate the switch and not tell her where the hell it is... It's one thing if she gets into an accident... It's another thing all together if it's was because she was watching a movie...

Yea man it's dangerous to be driving while a movie is playing or if you're messing with the nav. It's better to keep all your focus and attention on the road, but of course we don't always do. I play movies, mess with the nav, text on my phone, etc. Just be careful.
