Default *** Fixing headlight condensation ***

Hi Team members,
I have a 2002 GS430 and my headlight has condensation on both sides. The car has never been in an accident. I am not sure why it keeps coming. I have searched this thread many times and i cannot find a solution. I am a newbie at this. Can this problem be fixed or do i need new head lights? Please please assist.......
