How can I set it so that the fresh air is always on when I start the car after I shut it off. It seems like it almost always switch to recirculation air.

No idea, but why you want outside air/car exhaust anyways?

I just want some fresh air in the car. On a cool day, I can leave the window up and when driving on the highway, the vent will get some of that cool air blowing in without turning on anything in the climate.

Maybe it can be set in the PRESET mode? Trying switch it whatever mode and set your seat memory, I know it remembers other stuff. Worth a shot anyways.

The dealer can customize the setting, per the attached doc, so that it stays in the mode previously selected when the HVAC is in Auto mode or the A/C is on without being in Auto.

But if you turn off Auto or turn the A/C off manually, the default mode is recirculation. I've learned to just punch the mode button each time I start the engine.

I think it has to do with the ambient temperature inside the car & your temperature setting.
I noticed high temperature setting (73F) gives Fresh Air, and low temperature setting (70F) gives Recirculation.
Auto mode and A/C were off.

Actually this is one of the settings accessible to the dealer to change. It is under the Air Conditioning Section, and it has to do with changing the AIR INLET MODE from Auto (default) to Manual.
I had it done today on my IS250 and it stays the way you select it regardless of the temperature or other conditions. So yes, there is a fix, it would probably cost 1/2h labour at the dealer.
I know I'm super happy to have it the way I want it.

Mine used to revert to whatever it was last set to but recently I noticed it hasn't... well, at least that's how I thought it has been behaving for years. Somebody busted my recirculation gear before I bought the car anyway so it's pretty much stuck on fresh air regardless of the setting and I 99% of the time I don't even think about it.
