When I took the tire off, there it was a pad installed 'inside out.' I ended up installing new rotors and shims and all seems to be working well now. Thanks!

I had the same and did a full cleaning of the caliper including the gliders. They tend to get stuck by the dust and dirt. It's a real pain in the ... if they get fully stuck so I ordered a revision set containing all rubbers and seals + I bought some break grease for the gliders. Sins I had one stuck I check them every 10.000 miles remove the gliders clean them and re-grease them, just to never ever replace the wheel hub again that broke due to the stuck break gliders.
