update: so i raised the low beams higher but that made no change in the voltage of the height sensors. but i did adjust the height sensor level on both the front and rear while running a live test on techstream. my front height level had to be adjusted all the way to the top (as far as it could go) while my rears had to be adjusted all the way down (as far as it could go). from the last numbers you seen on that data list, i was able to move my front height voltage from 0.87 to 1.27 and the rear height voltage from 1.44 to 2.12. the numbers are still not near where it's supposed to be but i believe that is due to the fact that my car is slammed (so i don't have much range unless i raised the car up).
ran a live test while driving around on the highway and the AFS light off never flashed again! so i believe i have fixed my issue. hopefully you have your issue fixed too!

Front level sensor was causing problem
Here is pics of my sensors on front.
After that I have noticed that small rod was detached from sensor body.
Bottom part as you can see was just hanging down .I did clean sensor and just pack with grease rod and put it in sensor.
That was my temporary fix Wich is still working until I figured out how to glue rod into sensor.

with this fix AFS light is off .
one tthing I have noticed is that horizontal movement is working great but when I go updill or downhill there is not movement of the light beam .So I am thinking that vverticcal movement of the light beam is not working.,
Question is : If horizontal movement works ok and vertical is not working how come that light AFS is not blinking.???

Found replacement sensor only
Hi all. This thread has been super helpful with troubleshooting the DTC code B2416 that I'm observing. I found a replacement sensor only on amazon for a reasonable price of $27.00 USD. You can search for:
AIMUMU HUI 8940830130 Headlight Level Sensor 89408-30130 Compatible with Toyota Lexus GS300 GS350 GS430 GS450H GS460 IS250 IS350 2.5L 3.0L 3.5L
This listing is only for the sensor. If you have any brackets that are broken, this won't help you.
I am going to guess that the sensor is the same for both the front and the rear height sensor. I bought two just in case I need to replace both the front and the rear. I'll let you all know if it resolves my code.

Hi All,
Well, the front sensor has a different part number (89406-30140 front, 89408-30130 rear) . They look identical but I didn't want to risk it so I bought the front one as well. Replacing both sensors fixed the intermittent afs flashing light for me. A few notes:

- Don't try to reuse the nut that screws on to the sensor. Mine were so rusted that the threads were trash. I bought replacement stainless steel nuts and washers from the local hardware store for less than a dollar. They were 6 mm coarse threads.
- Because of the rust, the nut that screws on to the sensor was difficult to remove. I broke off the end of one the sensors with the nut still attached. The other sensor did not break off but you will need to use needle nose pliers to hold the stud of the sensor in place as you remove the nut. Otherwise the stud will just keep spinning. It's almost like you are replacing sway bar links.
- To install the sensor, I found it easier to remove the frame bracket that the sensor screws onto. They are held on by two 10mm bolts. With the bracket free, you'll have a better view of aligning the height sensor linkage to the sensor. I did not unbolt the linkage at all.
