A friend at body shop helped removing the bumper and we found that tabs holding the sensor were broken and sensor got rotated a bit as well.
After looking at how another sensor was installed, my friend just used JB Weld to put problematic sensor in same way as other, now everything is working fine.
Glad that it didn't turn out to be a bigger problem and I don't think we would have known the root cause without taking bumper out.
THANKS a lot for everyone to chime in.

Glad you got it fixed and didn't even need any duct tape or bailing wire (a.k.a. 'haywire') to fix the sensor that literally went haywire on you!

@bclexus Thanks for always stepping in to help with your wealth of knowledge on Lexus cars.
Just for everyone else's knowledge, there are two plastic vertical slots (one at 12 and other at 6 position on clock) and somehow both were snapped off and broken pieces were on the sensor. To find out which way the sensor should be mounted, we looked at other sensor and found the wiring harness should connect to right side at 3 O'clock position when looking at the bumper from inside. If sensor is rotated but still in current place, it still gives issues as @bclexus also mentioned.
Those plastic tabs are very flimsy and if something hits bit hard in sensor area, those can easily be broken. Even one out of two stays in place, it will keep sensor aligned properly but when both are broken then sensor may get rotated.
