Gentlemen,
Ive searched around and can't quite find a solution to my question. I've also called the dealer with my part number and they are asking me to point out on the diagram which part it is, but i can't seem to find it. Hoping that someone here can help as I feel illiterate at this point.
The part # is 063800-1991. It is an actuator and i will include photos of it to help anyone willing to help me identify this part specifically as there are like 10 of them on the car. I can find the 063800-1991 12V but does the second set of numbers/letter matter? i can't find a 12Q. The dealership couldn't help me identify it just off part number so they sent me a diagram to point out which one it is but i couldn't pick which one it was off of the illustration. I'll include that in case someone else can spot it and knows which one it is, or if it is even the right diagram.
To piggy back on to some other posts in case anyone was curious what the problem is. My A/C is working but its a crap shoot of what vents the air is coming out of. It says it will be coming out of the dash vents, but it will come out of the feet instead, or it won't be blowing air at all. Its truly random at this point. My mechanic has identified the 2 parts, #1 is the part listed above, and #2 is the 88650-30U80 blower motor controller module. Hopefully these 2 will fix the issue as long as i can get the part above identified with the correct # to tell Lexus so I can get that part ordered.

That's a Denso part number, see here for Lexus part numbers

I think its 87106-30560. it reflects the same denso part number but the number to the right of 12V is different, is that more of a serial number/production lot or does it need to match the exact one i have?

Not sure. My guess (emphasis on guess) is either is compatible. What makes you think your servo is bad?

the B1454 code that won't clear. Air outlet damper cool control servo motor circuit on the passenger side is what is associated with that code.

Is your car Japan or U.S. made? Don't know if that's a thing with your model and year. If you look at the parts listing it shows J and D version J is Japan D is Domestic meaning the United States (I think, could be wrong). What I don't get is how you are supposed to order the correct one, or if there is only one version available. I asked a parts counter guy at a Toyota dealer about this didn't get a clear answer.

yea, this is adding to the difficulty of finding the correct part.
I actually just realized that part number up top i listed isnt right. its actually 87050D - Damper servo sub-assy, air conditioner radiator NO.2. 87106-30610, the illustation says type B but the one i have here is a Type A so back to the drawing board over here.
What I don't get is how you are supposed to order the correct one, or if there is only one version available.

It's a servo motor that controls the HVAC damper doors (a.k.a. blend doors) in the ductwork of the vehicle. There are a number of servos in a typical vehicle.
Here is a similar servo with the same Denso prefix:
If it requires removing the dash it's a PIA. Some might require removing part of the A/C and that too is a PIA not to mention the disassembly and assembly of all the servo motors.

According to the parts listing there are two 87106-30610 can you swap them see if it changes anything? Swapping servos should give you code B1453. I looked up some other models of this era they all show type B (no type is listed by I can tell by the diagram picture) so maybe type B is functionally the same. Guess I'm too dumb to figure this out.

Not sure how your mechanic pulled that part out, but if they disassembled the dash, it might be good to regrease the other servos while they are accessible. Part of the TSB solution is to update firmware on the HVAC which then enables a procedure that takes a while that distributes grease all the way past the typical range.
To me, that sounds as if part of the problem may be in the gears' lube drying out in the part of the gears(s) that is always used. Granted, I'm not sure what lubricant is used.
I did see some other fix for the older GS that involved cleaning some electrical contact film that helped determine position.

The number on the old part is a manufactures P/N, replacement part numbers are different.
Fault B1454 is for the servo motor in location 7 in the diagram. Image "E" is a view of the passenger side of the HVAC case.
There is a reference box to identify type and build date of that servo motor.
All servo motors can be replaced without removing the dash.

This is a huge help. Correct me if i'm wrong here Nevada, but 7 references 87106-30530 which was produced 2011-2014, and 87106-30610 is 2014- onwards, and they are interchangeable even with the little cog component difference?
