Arggh, Bluetooth Fail

I received an automatic Android upgrade on my HTC Incredible and re-registered the phone on the GS w/ML to see if MAP was present (it was not). It registers fine, transfers the contacts, and works. However, when I turn off and restart the car, the car and phone won't reconnect. Deleting and re-registering the phone yields the same results: works at first and on next restart, nothing. Suggestions?

This sounds like a similar issue I had with an HTC Amaze BEFORE they updated it to Android 4.0. The problem with Android, being open source, it allows the carrier and manufacturer to add things and make changed, such as proprietary bluetooth stacks (ie. to prevent unauthorized tethering). It is really tough to tell what is causing the problem sometimes. If I come across this issue here I'll try to get back to you with what I find out.
BTW: I have an HTC Sensation and it works very well (including MAP).

I tried it again after removing the battery from my phone with the same results. I have an appointment with the tech specialist at my deanship on Thursday afternoon.

When you are in bluetooth settings, under bluetooth devices, long hold on the device you paired with, does it give you any options? Since it already connects, just doesn't reconnect, that might not be the issue but worth a shot. The music pairing initially caused some issues with our GX but it magically went away one day - it used to keep connecting and disconnecting.
Try unpairing from your phone as well.
Last thing you can try is after unpairing it from the car and phone, try renaming the device and re-pairing.
I'm just guessing here but I had some frustration with BT before as well.

When I arrived at the dealer, the tech specialist was at a class at the Toyota store across the parking lot and told the service manager he would be back in 10-15 minutes. The service manager took the phone and started playing with it while I went in to get something cold to drink and talk to the sales reps. When I came back about 10 minutes later, he wasn't making any progress so I instructed him in the steps I took to unpair from both the car and phone and start over. He did the same steps I did and it worked - and continued to work after he turned the car off and back on. I got in and turned the car off and back on and it continued to work. We don't know what we did differently, but it appears to be working - for now.

I've got an iPhone 4G. It works pretty well, but I have had a couple of minor issues. If I grab my phone and activate certain apps, the BT Audio goes off. Nothing major, for me. I just restart my iPhone music app and it comes back.
