I would try your dealership as I don't think you can purchase the screw alone. I think you would have to get a whole new housing.

You can buy a used led fog lamp unit on eBay for $20 including shipping:
Here is a listing for a RX fog lamp, same one used in the gs, you can easily pick the screw/grommet off that.

The ES, GS, and RX use the same fog lamps.

Otherwise Lexus does not sell it separately you would have to buy a new one.
