I went through almost that same process as you. TSB, etc. Resurfacing rotors and using the new updated pads have been the only remedy that has worked.
It was in the low 30s F this morning. And there was zero noise. I drove my wife's car around for a couple of hours, and there were no squeals/squeaks, nothing.
Step 2, was probably the repair procedure, on which parts to swap out for. They only printed those 2 pages for me.

- Remove any transferred material from the current pads, especially if the rotors are glazed over.
- Correct any imperfections, even the slightest, and have a fresh surface for the new pads.
- Because the rotors are relatively new, they are better suited to resurface them because they likely have a lot of thickness left. (Older, more worn rotors should NOT be resurfaced below minimum recommended thickness)

Update:

We've driven 400 miles since the pad change. Keeping my fingers crossed.

It was about 30 degrees F this morning when my wife left for work. No squeal. Not even on the initial backing out the garage.

Heads up folks. Here in Los Angeles I’ve been calling different dealers and they’re at least $154. One dealer had them listed at $194. I was told that they’re on back order. Yet in some sites when I just search Google for the part number I see the price listed in the $80-$90 range.

Off topic: I want to try to replace the pads myself, doesn't look hard. My car is lowered on coilovers and is very tough to fit a standard jack underneath. Anyone can recommend a low-profile jack? Maybe something from Harbor freight for reasonable price <$150. I figure I might as well invest in a nice jack so I can take off my wheels for cleaning in the future and adjust my coilovers myself.

@tangierc I just did a Google search and click on Longo Lexus link for each of p/n: 04465-30540 & 04945-30340. Within 5 minutes it was ready for pickup. I just return from picking up at Parts counter. The guy said these are "new and improved pads" to get rid of squeals...we shall see. Here are some pictures of different pads I have for comparison...sorry I don't have pictures of the original pads. I ran Centric Posi-Quiet with the OEM 2pcs rotors for 2 years with very little squealing. I'm running Akebono pad now with aftermarket StopTech rotors, it's squealing very badly. Notice the different material composition and the new pads are more squarish. There is also an angle trim on the corners. Hopefully with these modifications it will rid of the squeals and squeaks for good.
