If you do a search, there are a ton of topics on this forum on the base (or luxury package) GS vs the F-Sport. Being a hybrid is just the same car with a different V6 engine that features an Atkinson cycle and combines power with an electric motor. Slightly increasing weight (200lbs), but gaining torque and a lot of efficiency. These cars are fairly bulletproof, so I doubt you'll find many issues.
Here are two previous topics on what you get with the F-Sport package (and what you lose out on):

The 4GS hybrid subforum can help you if you wish to read specific information on the hybrid model: https://www.clublexus.com/forums/hybrid-technology-242/
G'luck with your search,

If you can find it the best combination is the 450h with the luxury package and to get the best of both worlds you can do what I did and change the front bumper to f-sport. The Lux 18 way seats are the most comfortable you can get for the GS and the semi-aniline leather is noticeably softer. I know because I've had both.
This is a luxury car with a nice ride and when I feel like it throw it into sport+ for a tighter suspension, tighter steering, and more sensitive accelerator pedal.

I have the same experience as @peasodos . My vehicle is also a 2013 GSh, but only has has half the miles as his.
Early on, I got the dealer to replace the sunroof seal on my vehicle under warranty (it was just causing a bit of noise when driving on the highway), but that's not a difficult at all to do on your own.
I also replaced a ticking fan noise in my radio/head unit: https://www.clublexus.com/forums/gs-...head-unit.html
^Not every GS owner seems to experience that, but since the hybrid is so silent when on electric power, you notice small things like that. It used to occur only in very cold weather for me.
Other than those two things, no issues to report if you following normal maintenance. I plan to keep the car for a long time because it is built so well. Also, the GS line is being discontinued by Lexus and there is no luxury, sporty 4 door sedan equivalent that Lexus produces -- unless you want a smaller vehicle like the IS.
All the best,

I will probably drive f sport next week since car is not available its on holiday the person who do the sale, for me the most important things are the maximum reliability... since in bg we dont have lexus dealership and all parts needs to be bought from abroad... which will take few weeks at least etcs...

Your reliability for a used car is mostly determined by how good the previous owners have taken car of it.
Best go for a single owner car that has a service record for every year.
common issues are not specific to the Gs450h. But I’d recommend that you check the voltage difference between cells and that the hybrid transmission shifts smoothly between the hi gear and low gear. It is a multi stage hybrid with B1 and B2 brakes operating.

Are you recommending checking the cells of the hybrid battery? If so, is there a procedure for the 4GSH?

So yeah I doubt you will have to ever worry about that unless your car is really high mileage(250k+). If the car is getting 32-34+ MPG highway then your battery is in great shape. At 111k+ miles zero issues with the hybrid battery.

You want it to be .2V or less.
0.20V = 100% acceptable efficiency
0.45V = 75% acceptable efficiency
0.70V = 50% acceptable efficiency
0.95V = 25% acceptable efficiency
1.20V = 0% acceptable efficiency
