error 3
I googled cd error 3. In one of the posts here from toyotanation here is what I found:
In Scion's, Error 3 typically means it can't read the disc or the unit has failed. Take it in and get it replaced. Description of errors for Scion CD players:
ERROR CODE DESCRIPTIONS:
Error 1: Dirty Disc/Cannot Read Disc (Clean CD as needed and try again.)
Error 3: Mechanical Malfunction (Replace CD player.)
Error 4: CD Player Overcurrent (Allow CD player to cool and try again.)

Here's another one which points to the problems and liittle solution other than replacment.

Interesting but it seems this is a common issue. Some people have taken the unit apart. Here is a posing on an audi. Seems to sum it up.
----------------------------
Hi saw no "reset" type button on the back.
Well,
I decided that paying Audi £300 wasn't an option, so, I stripped the unit down!!!
I did this WITHOUT breaking the seal!!!
I took the front end completely off, released the CD magazine, pulled it out from the head unit which then enabled me to look directly at the CD's in the unit.
I could see that they were not all in-line so with a very small screw driver I simply pushed the edges of the CD's together which in-turn snapped them back into place!
I'm not sure how I would have got the CD's out if I had needed to, but thankfully it was not needed!
So, the unit went back together, back in the car and hay presto. it works fine!!!!
I hope others read this as I'm sure there are many prople who just shell out the money!
I must point out though, it is not a job for the "Heavy Handed"
-----------------------
there are repair services on ebay. I would take it out and try to see if I could get to the cd's.

Thanks Mansito I think I may have to split open out the system to check out if the magazine is having a problem, if it so, then i may order one. New one cost 55 bucks only. It is better than replacing the whole stuff. As you have quoted from the toyota nation, code three refers to Mechanical, then for sure this is the magazine tray problem.Any I will let you know once i get this job done.

Does this work on GX470 2003? I tried the procedure but it did not clear out my error 4. How long do I need to hold down the power button of the radio after I turn the car on.
