If you rotate at America's tires, they will rotate the spare in for you.

One thing else not mentioned, when someday, way in the future, and the limit marks are beginning to show on ones tires, it's a lot cheaper to replace four tires instead of five.
I say this because I'm running LT285/75R17 BFG M/T KM2's at $324 each. I'd much rather only purchase four instead of five when the time comes.
Although, if my tires were well worn down and had to use the brand new spare, I don't think I'd want to put a lot of miles on it regarding diameter difference.

On that note, here's a personal experience: My wife and I purchased in 1992 a new Toyota Previa All-Wheel-Drive mini van (great van for the time). At just a couple of thousand miles short of the warranty running out, we had a flat. I had custom 17" wheels with low profile tires (looked great!) but still had the stock spare. Got a flat and had to use the spare which was a slightly smaller diameter. Ended up winding up the entire drive train and caused thousands of dollars of damage to (if I remember correctly) the transfer case and drive shafts. Fortunately as mentioned above we had a little left on our warranty and Toyota picked up the entire tab!

Question: anyone know for sure per Lexus documentation or otherwise if a slightly different size will wind up a drive train as mentioned above?

When the tires are rotated (at home) does the TPMS recognize the new location?

My 2018 Tacoma does not care since the same sensors are involved, but the GX?

A bit of a necropost but I'm experiencing an issue with this too. Just did a rotation on my tyres [Front and back swapped in an X pattern, as is standard for 4X4s]. The rear right tyre had a minor air leak from a previous repaired puncture, but I had that plugged up while I was there. I aired up the tyres afterwards to 35 PSI [assuming the electronic air pump at the petrol station I was at was accurate]. The TPMS light did not turn off, though. My GX is a 2017 model. Anyone know what might be up?

Make sure the spare is inflated to the same pressure... it has tpms transmitter too.

Yep. That turned out being it. Took the car to the dealership to be sure, told them the issue and they checked the tire pressure on the spare. Resolved it fairly quick.My car is now sorted for the foreseeable future.

2020 gets PSI info on the dash but for 2010-2019 you can also see individual tire pressure via OBD and your phone.

I haven't tried with other programs but this setup does work.

I purchased new BFG KO2s recently and plan to rotate them in about 3k miles. Should I just purchase a fifth at the time of rotation and perform a five tire rotation?

Interesting. I did not realize Lexus tire rotation recommendation. I rotate mine without spare, simply because too much trouble to include it while I know I get the most out of the tires by including it.

I do not see the rationale why we rotate non-directional tires the why Lexus recommends. I rotate rear to front, front cross to rear. I get approximately 80K miles out of Michelin LTX with even wear. I got rid of the tires because of age as I do not put a lot of miles on one single vehicle.
