So for anyone else with this issue, turns out it isn't a loose connection. The issue came back after a few hours of driving. I ordered a replacement ECU from a wrecked GX and am getting the same results - it works intermittently, but out of nowhere will throw that error message. My Carsoft Toyota scan tool detects the ECU but says there are no error codes. The only thing I can think of is an issue in the wiring, and it's just not something I care to dig into any further unless anyone has any ideas?

I know it isn't throwing codes and I haven't looked at connector recently but have you thought about swapping sensor locations to see if it problem follows going from the middle to a side location or back? It might be easier to swap middle front to middle back as the side ones are differently shaped IIRC.

Sorry..... I forgot to look further up in the thread at the pictures you posted.
I notice your OBD reader says "Intelligent Parking Assist" ---> Without
Is that a detection from a vehicle or user prompt to select? Just curious if your reader isn't fully reading the ECU.
I know OBD Fusion sees the KDSS ECU but no PIDs within it.

Like you saw FSM really only lists replacement of that ECU you already replaced so may be wiring.
Another thought... I think that dash message is the generic error that shows up for all issues even if it was just one sensor. Seeing that the nav screen is showing front center might still be worth looking at swapping some sensors front to rear.
EDIT: I stand corrected. It does show specific sensor in FSM on warning in gauge cluster.

If you have a Techstream install setup you may want to see if everything is being detected correctly in the data list for ECU. This may help isolate the area of the system to look at if it is wiring.

Rain causes front emergency sensors to go haywire on my 2019 RX450h Fsport.
Yesterday I drove in the pouring rain for 45 minutes and my sensors did not react through the storm. I was quite surprised and said to myself it's just one of those things. Thanks everybody for your responses b a r r y s l

Please help! 2011 GX460 - Which parking assist sensor is this?
New to the GX world and the forum as well. Just trying to troubleshoot Check Sonar System and I am slowly finding out perhaps its not my dynamic cruise control that is a problem but rather one of my park assist sensors at the front?
Please see the attached photo. I understand that there are 4 sensors at the front of the 2011 GX460s but the error message/picture shows a sensor at the front DEAD in the centre.
All I see in the center of the front is my front camera which is functioning. Is there another sensor there that I am unaware of?
