Masboy..you are doing it wrong...it is anti clockwise for loosening and clockwise for tightening.
Make sure B4 u remove make some notch marks on old disc and the axle and then remove the disc. And while you align the new disc , the position to find is where the disc has the minimum runout, that is the place to align it. And make the torque marks on the rear of the disc. And I deeply apologise for the torque figures.
It is Torque 112nm ( 1.1242kgf.cm,83ft-lbf).

Use a Fibre Mallet, iso of a wooden one, give some hard knocks around the Rotor few times, they will come out, as they are bit rustic, Only make sure the knocks are hit each time, the next knock should be diagonally opposite end, to have the balance of the impacts done. I had the same issue,For me it took around 10-12 mts to get it out. Dont worry, When you refitting it, make sure you grease the areas which are rustic ( Preventive Maintenance is always good).
Here is the DIY which I had compiled it, Hope this helps, here it includes the Rotor Skimming.
cheers

thanks a bunch for the PDF !!
So hit the rotors from the front side ? is that ok ? then just hit the diagonal opposite end from the first hit and so on ?
I have a rubber millet, that should work, right ?
I'm kinda confuse on this part from the pdf :
Here ensure to check the disc thickness by just running soft fingers over the face of the rotor.
Here if you feel any fine lines, then you have to skim the rotors. Here I had to do it. The Toyota
method is Inspection ensuring standard thickness 28.00mm and Minimum 26.00mm. so you
need to skim at least 0.05mm outer edge from each side of the rotor. And that is what I did.
Note: Before you skim the rotors ensure to mark the disc of each wheel as a matter of
precaution..

Ur right just hit it ! On the front side only, and Skimming was done, as mine was had fine lines, And that I why I ended up doing it. Now coming to Skimming part, The Rotor thickness normal is 28mm, When you skim it you should reduce the mm not below 26mm. As the rotors would crack, so here is your benchmark to check how much you could run down on the skimming part. What I removed was 0.05mm, from the original thickness of 28mm. So I am still under the Toyota Specs. Have a close inspection of the rotor and If you have vernier calliper check the measurements around the rotor and take the decision whether you want it, or else you could replace the pads and run for another 6-8 months and then go for skimming.

Here is the TSB for the GX Inconsistent Brake feeling, also the replacement method is mentioned, Get the new shims and Anti rattle springs while replacing the pads.
