i don’t know the state of the engine or transmission, but based on driving it these past three days it all feels to be good and working as it should. The car fax the dealer gave me showed the car received frequent oil changes and maintenance tune ups often. The tires seem to be around 3/4 life and we’re just rotated and aligned, the suspension feels good. I do use the ac, however it’s on a low setting.
Planning on doing plugs, air filter, clean MAF and throttle body, and fresh coolant at 100k and hope it helps. I also need to check my battery.

Pretty old thread. Pretty but not very old 450H- 2015 luxury with 44,000 miles. Only one long multi-day highway trip. Average mpg since new 27.5. Average speed since new 28 mph. Hand calc mileage difference is in the noise + and-.
Do notice better performance and slightly better mileage with top tier premium than top tier regular. 85% of tile I use premium. Mid grade when cost differential is outrageous, which it often is. Differential between regular and premium is always at least 25%, often 33% or more completely offsetting any mpg savings versus gas equivalent RX. Given that it takes a lot more energy to manufacture premium, it probably is not really a good ecological solution based on fuel. However the 450 excels in comfort, power, and things like brakes and tuneup parts.
I would have replaced with a 22 450H if I could find a luxury that was not black inside/outside or uncomfortable F-Sport seats (or all 3!). Opted for regular gas 350RX. In 10,000 miles average speed is 27, mpg is 22. Again minimal difference between hand calc and info center reading and same driving conditions and habits on mostly flat land.
On GasBudy I see spreads between regular and premium continuing to grow, and stations having much larger station to station spreads for their premium versus station to station spreads for regular. And if you do not pay attention, cheap regular often means rip off on premium.
I did not and would not wait and jump on a 23RX 450 because it will be a first model year for the power train and some other features. Hopefully the 4-cylinder turbo engine will carry the weight flawlessly and won’t suggest or require premium. Only time will tell.

I think it's hard to get 30 mpg at 75 mph. Best I can get on my 3rd Gen is just over 40 mpg (UK gallons), so 30 mpg US. That's maintaining a speed of 60-65 mph though. It drops noticably if I go above 70.

I have a 2010 Lexus rx 450H for one week now and I want to share my experience. The car has 128.000 miles and is front wheel drive. In the city if I am accelerating slowly I am getting 30, 32 MPG. As long as I keep the speed bellow 60, my mpg has a average of 30. But on highway the mpg is horrible unfortunately. At 70 miles per hour I get a 24 mpg and at 75 I get under 20mpg. Now I am more excited about driving in town, which I never believed will be possible. From what I read on this forum this is a normal gas consumption but my question is: there is no possibility to not have that brake once I am taking my foot off the acceleration?
