Help. Want to buy a 2010 450h with 145K miles.

Sounds like you're looking at mine, similar mileage, and also rear shocks, water pump, brakes each replaced once. That's all I've ever done to it other than fluids and filters maintenance. Oh, I also had the rubber oil line changed to the metal one too (was free in Canada).
I'm still driving mine daily, still gets super fuel economy when I'm inclined to do so, and generally I'm still happy with it although sounds like mine has more wear and tear than the one you're looking at. I wanted a model Y, but wife is resistant to the expense at the moment so we continue on as is, its costing us next to nothing at the moment.
That's a lot for a warranty, I would be hesitant myself but I don't think you mentioned how long the warranty is for. If it was a good long period, well, maybe. I'd say there is a 70% chance you won't use it at all depending on how long, and 3.5K will buy a lot of repairs, maybe not a major system failure though.

less than 100k miles? Yeah you’d be ok getting the RX450h and it lasting a decent amount of time
over 100k miles? You’re better off getting the RX350

Well I ended up buying a 2010 450h with around 115k miles. Each and every service performed by a Lexus dealer and a brand new hybrid battery replaced at 105k miles. I'm very happy with my purchase.

Just as an aside, I bought a 2010 RX450h nearly four years ago, it's now at 105,000. Nothing goes wrong with it, just oil changes every year.
I called the local Lexus dealer to inquire about the oil cooler question. He looked up my car, said "You have no issues, the oil cooler thing was with a fitting which rarely could leak, Lexus extended the warranty to 150,000 miles, but in any event your car is exempted." The car does have the towing package, which includes the oil cooler, so I don't know much about a "rubber hose."
The service tech also said the next major thing was plugs at 120,000. He said Lexus could do it for about $650. I think I can get that done for far less at the Toyota dealer or perhaps a Lexus specialist. YouTube videos show it can be done by a backyard mechanic, but it's a bit involved to get to the rear three plugs.
As for the traction battery, I've read that they rarely fail, even at 200,000. We'll see. Great car.

Pay someone, even the dealer, but any shop can do this repair, to replace the rubber oil cooler hose with the upgrade metal lines. Total cost is less than $300, probably less than $200. I did the repair myself, but do most of my own maintenance and repair. The part itself is less than $70 and is available either OEM Toyota or aftermarket.
If the rubber lines fail while driving it will have spewed out all of your oil and your engine will be totally destroyed by the time your oil warning light appears and you are still deciding where can I pull over and shut down the engine without getting killed.
Lexus will only pay to replace a leaking line. If it fails and wrecks your engine they will find a way to deny coverage.
