I can be anything from having an intake system that brings in more air to minor exhaust items that let the air flow more efficient.

This argument is actually comparing apples to oranges.

Actually the engine should be rated at 314 HP had it not been for the new SAE standards. See the below article for an in depth discussion of the 2GR-FSE (If you look at the charts comparing the DI with the combined DI and PI you will see the hybrid injection sees a 7% gain which, oh by the way, equals our magical horsepower difference):

Actually running lean will produce more power. Although short lived the power comes on by leaning the mixture.
