well.. lexus is fixing it for me already, i'm using the lugnuts that are made specifically for volk gtc's, they are open ended and they curve in... i don't really kno how to explain it
i installed the wheels MYSELF, i'm pretty sure i didn't tighten one of them.. because basically all my other wheels are fine, even the rear passenger side wheel, all the lugnuts were tight on the wheel that the lugnuts fell off too, except 2, i'm guessing one fell off.. and it caused a little vibration and the other one was just loosening
the stud wasn't in the rotor when i discovered i had no lugnuts, it was loose, it was pushed in a little bit, then bit by bit it just went in from all the movement i guess, cause i had to drive my car to a local tuner shop to pickup 2 extra lugnuts.
when i got to that shop.. my stud was in my rotor.. thats when i declared my car un-drivable and towed it to the nearest lexus dealer..

ehhhhh torque wrench always helps haha
i always carry one in my car but eh, technically if you used oem tools, it should be alright cuz its not like you used an aftermarket tool to f*k up your car
does that make sense?

Wait, so what exactly happened with the stud? Did it just fall back, did it break or did it fall out completely?
The lugs falling out, is probably because you didnt retorque the lugs after driving 50 miles. I would also suggest checking them after 10 miles.
If it fell back, but didnt break or fall out, you coud have just taken the wheel off then used a lug nut to pull the stud back. I did that on my 3000 many times. (If the lug was spinning with the stud, you can use the factory L iron, place it on the lug and pull back while turning it)
If it broke, you were SOL
If it fell out completely, then I cannot see how they would have to take your entire brake asspembly apart. If it can fall out, then it can go in as well.
