I can't believe there is still argument after some people have mentioned that the pressure distribution is what causes it....
No matter what you do, when you're going at ~40mph+, the pressure made by the stream of air moving around the vehicle will create pressure variance and when you open one window, you will notice it! Mainly because by opening 1 window, you're letting the air flow in-and-out. Imagine if you were holding a peice of silk towel from B-Pillar and opened the window....the towel will move back-and-forth, which is a visualization of air pressure change. That vibration is what's causing your ear drums to move and therefore, you'll feel PAIN!!

I have owned several American and Japanese 4 door cars, and been in dozens of others, and have never felt as much ear pain as I do in the Lexus{with one rear window down}. Here is my theory: the Lexus is sealed so well, that with only one window down, there is no depressurization. I'm sure all the other cars I have been in did not have as good of a seal, so air was able to get in somewhere else to depressurize the cabin, thus less ear pain.

I thought I was going bust my tympanic membrane when when my child rolled down the back window at about 40 mph. I now use the window lock when they are in the car.
