Dealer called today and they are going to do the piston ring fix. I was a bit leery of this as the noise was minimal but they said it would only get worse. Will post when I get the car back.
Based on your description it sounds like piston slap, they all do it cold its normal operation

Possibly, but I don't think so for several reasons:

1. The car didn't do it when new (started noticing it at around 35k).
2. No other IS250/ES350 I've driven makes this noise.
3. I don't think Lexus would replace an engine (essentially) unless they were confident there was a problem.

Let's see what happens.

apparently I've got a new shortblock

Sooo I got the car back.

They replaced the water pump

Sounds good, starts right up as usual. Power feels the same. There's some new noises though. A higher pitched fan/pump kind of of sound (new water pump maybe?)..

One major difference is that my brakes now suck. They used to be so touchy and I could NEVER get it to the floor, now... I have to push real hard on them to get some real stopping power AND I can hit the floor.

I noticed the same thing after the repair. Bleeding my brakes this weekend so hopefully that makes it feel better. My brake pads are still pretty much new just changed them out about 7k miles ago and bleed the brakes as well then.

Did you have the block replaced? Or just the rings/etc.

Yeah If your brake pedal is going to the floor it sounds like the brake lines were not properly bled.

You ought see to that. Quickly.

I sent the service adviser an email letting him know about the problem so at least he knows what happened. I'm sure after all the work they did on my car they were just ready to get it the F out. Even though that's no excuse for not bleeding the brakes, I might as well just do it myself tomorrow.

The brakes are not touched they stay in the car when you drop the subframe w drivetrain... Its weird that it happened to u.... As for the short block, its not part of any bulletin, so there had to have been something more to it.. I have the bulletin in pdf format

I finally opened my engine to check the valves n used a bore scope to see the pistons ill post pics tonight. The valves didnt improve that much but the piston domes are totally clean 66k and no trace of carbon! I ran 16oz gm to engine clean, 32oz toyota engine clean + 16oz washer fluid @ 4k rpm. In the last week my fuel trim is down to 2.1%... damn near perfect.. I doubt ill be seeing any misfires for a long time

The bulletin mentions replacing the short block if there is any scoring on the cylinder walls, or there is not enough visible cross-hatch. It says not to hone the block, but to replace it.

yes, there was scoring on #3 walls.

can you tell me what else comes with a short block? Does this mean all the pistons/crank/etc were also new? Or did they remove old parts and use them in the new block ? Stupid stuff like Sparkplugs are not listed on the invoice.... but I'd like to know if they were changed.

I should have asked my adviser but he was busy.

Short block is the block, crankshaft, rods, and pistons; assembled.
Bare block is the block alone.
