When I bought my car it had only one set of keys as well. I bought that key fob and the master key that goes along with it. Don't forget the master key (http://is.sewellparts.com/accessorie...2006/1864.html), unless you want just the key fob. I took it to my local dealer and had them program it, which costs $50 at my dealer for the programming and they cut the key for free. You can call yours ahead of time to ask how much they charge to be sure.
My dealer kept telling me they would only program it if the key was bought there. I brought it in wrapped in a bubble package with the lexus sticker on it and said my brother bought it for me from the dealer. They programmed it with no problem. The reason why they started doing that, I guess, is that people would buy these keys that were previously programmed, which would lead to problems when trying to reprogram for your car. If you buy it from Sewell you should be fine. Call your local lexus dealer to see exactly how much they charge.
