While what happened to you is pretty scary I've had the V2 spacers for 3 years now on my front wheels without any problems. I take them off every winter when I put my winter wheels on so I get a chance to inspect the spacers twice a year and no problems so far. Your story does make me wonder if I want to put them back on in the spring.

They push your wheels out more and widens your stance a little bit

scary stuff right here! When I used to make spacers for my own self, I would always ALWAYS make my spacers wheel and hub centric.

I know a LOT of people who run spacers and nothing ever happened, either you just had some bad luck or the last person to install your lug nuts over torqued them because the studs on spacers aren't as strong as stock studs

Nope, which is why I'm not running spacers bro! Just stating I know a lot of people who've been running them for years... so obviously there's some people doing it right and some people doing it wrong.

Hmm.. Sorry to bring an old thread back up.. but I was about to buy some more spacers and ran across this thread..

So, this is what bothers me with this picture.. There are too much threads left on your broken stud on the spacers... Which lead me to think you might of had wrong type of lugs for your wheels or another spacer on top of this. I just checked with my V2 15mm on one of my old SSR and for my lugs to sit flush with the wheels, I have about 4-5 threads from the surface of the hub. On your spacers, looks like you used only 4-5 thread to mount your wheels. This could of been the issue where you were only holding the wheel (weight also plays big) at the edge of your studs, creating too much stress.

Of course, this is just my observations.
