You can give a try to explain the rough idle on the engine and the dealership would probably replace the piston and piston ring. From what I heard, all Lexus dealers requires to have check engine light associate with the carbon build up issue for the replacement on piston and piston ring, or short block if there are any scratches on the walls.
The engine is still ok since Lexus use the same engine in the 2012+ GS250 in Asia and UK region. The same engine has about 8HP more than North American region. The emission control in North America detuned the engine which it's a lost of 8HP in the IS250.
If the engine has serious flaws or problems, Lexus wouldn't use the same engine on the newer models.

Not wanting to turn this thread into a carbon build up discussion, but I think that the emission controls in NA have something to do with the carbon build up issue as recirculating exhaust gases into the combustion cycle is bound to create contamination in the combustion chambers.
I had an Infiniti G20 with the SR20 engine that was notorious with clogging out the EGR valve and tubes with gunk from recirculation gases. Every 3 months, I would have to take the tubing apart and clean everything with carburator cleaner. That car lasted from 1991 to 2008 as ours, and then our mechanic bought it for his daughter and still going.

That is really not an indication of reliability. Lexus may just use the same engine if it is cheaper to deal with the expected warranty rate than developing a new engine at that particular time for that particular market.

However, the engine is used on a various vehcles since almost 10yrs ago and on to the newer models. I would think the engine's reliability shouldn't be as bad as we thought. Also besides from the carbon buildup, I haven't heard/read any other issues.

well, i read recently about pistons being replaced and idling being "rough". i dont really know how to pick out "rough" idling. would indicators be noises and high idling???
