It's good you caught the lean condition because very bad for your cylinders. Since you are using E85, your problem could be the injectors can't keep up with the amount of fuel that needs to be delivered at low rpm/high load, a classic problem running e. Be careful because I've seen injectors going bad as well as thrown rods with long term E use, at least with 10th gen Civic Si's (thrown rods would be less of an issue with the IS350's 3.5L)

No lean issue now with the normal 2-1-2 pipe installed. But still don't know why a H-pipe can cause it to be lean

It probably opens your exhaust flow allowing more air/fuel to be pushed, and certain conditions (low rpm/high load) the injectors or fuel pump can't keep up. I'm just guessing. Try just pump gas and see if the condition persists with the H-pipe. If not, it could be fuel flow issues running E85 when the H-pipe is installed

Any chance you friends hardware / software can monitor valve timing? Speculation is dumping cylinder pressure by retarding cam advance making lots of overlap effectively recycling its own charge gasses right at the intake/head level.

Those are some very interesting findings. If you had an EGT sensor on a few exhaust ports you'd see the temperature drop swiftly during this lazy period.

I wonder if this is a better solution?

Since he is using E85, the issue is most likely fuel delivery. I've seen this a lot in the Honda community when trying to achieve higher WHP using ethanol (e40 and up). The fuel pump and injectors can't keep up causing lean conditions which cause misfire and or power being pulled in high load scenarios. I had a friend who had a 2017 10th gen Si, he was pulling around 270 whp, and due to the combination of running a crappy aftermarket airbox (Injen), running E40 and probably a bad tune, his fuel trims were way off and he was running lean and threw his rods...literally. It happened at low rpm/high load. A few people have tried to get around the fueling issue by using meth injection, which for some who didn't build out there engines ended badly.

But also you are right, ethanol tends to have a cooling effect so there is probably a temp drop in his exhaust ports
